include-what-you-use wanting them to be removed.

The 'arena' allocator does not take memory alignment into account, which
can result in crashes at -O3 with gcc 7.5 (presumably because of SSE
instructions).
This accounts for alignment issues, fixing the issue.
Also do various cleanup in lib/allocators.

GUIObjectBase is made a IGUIObject* to avoid including those headers
un-necessarily. Subsequent diffs ought to clean up the various of
pointers for that with a similar type with reference semantics.
Also:
- Add standard C and C++ headers (mostly cstring for memcpy, string and
vector) where needed.
- Swap out some includes for forward declarations
- Clean up un-necessary boost includes in precompiled and other headers.
- Clean up precompiled headers, including fewer things.
- Move ACPI to the windows-specific folder as it's included there only
and mostly specific to that platform.

compulsory custom deleter). Its features are not really used, it
contains outdated atomic calls, and it blocks D1511, thus it is removed.

This fixes the problem where passing a non-ASCII string to
debug_printf(L"%hs", s) caused vswprintf_s to fail on Linux (because it
doesn't know what encoding the char* is meant to have). Now debug
messages will remain as UTF-8 until they reach the OS.

Once 'delete' is called on an object, that object no longer exists, and
accessing its member variables is undefined behaviour. GCC 4.9's
optimiser recognises this, and eliminates any writes to member variables
inside the destructor, since it knows they cannot legally be read later.
BoundaryTagManager relied on ~FreedBlock resetting its memory to 0, so
this optimisation broke it. Replace the placement new/delete with plain
non-magic Setup/Reset functions, to avoid the optimisation.

replace array-of-pages data structure with demand-committed VM => allows
simple allocation of new resources without having to search for the
first free index. I suspect the cause of multiple reported bugs (refs
#860, #899, #915) was a race. Most issues are fixed, but the pool's
freelist also needs to be made thread-safe and lock-free. this has
performance and 64-bit portability implications, so I'll leave it for
later (Pool is due for some serious refactoring anyway).
