Fix includes in source/ps
Make include-what-you-use happy with some files in source/ps and fix
what needs to be fixed.

Delete source/ps/FutureForward.h as it is no longer used.

Dissolve external_libraries/openal.h
This header was used to overcome macos openal vs openal-soft header name
difference, for the migration to openal 1.1 and to pass arguments to the
linker on windows in the past. None of this is used anymore so dissolve
the header.

Also fix related headers so one can run include-what-you-use on the
changed files.

5ee8354e99 Fix windows SEH hook when crashing in an std::thread
Follows 107d3d461f and other 'pthread->std::thread' diffs.

Windows uses Structured Exception Handling to allow reporting errors
(both C++ and hardware) nicely. This works by wrapping the code in a
__try __catch block.
The pthread wrapper did this automatically, but we now need to do it
explicitly for std::thread.

369c2e8801 Further header & precompiled cleanup, fix no-PCH builds.
GUIObjectBase is made a IGUIObject* to avoid including those headers
un-necessarily. Subsequent diffs ought to clean up the various of
pointers for that with a similar type with reference semantics.

Also:
- Add standard C and C++ headers (mostly cstring for memcpy, string and
vector) where needed.
- Swap out some includes for forward declarations
- Clean up un-necessary boost includes in precompiled and other headers.
- Clean up precompiled headers, including fewer things.
- Move ACPI to the windows-specific folder as it's included there only
and mostly specific to that platform.

ba736916fc Clean up ThreadUtil, use standard C++11 constructs instead of custom ones.
ThreadUtil shipped a scope lock and a mutex implementation, which can be
removed since we now have these in the standard library.
This lets us clean up this header which get included everywhere (through
profiler includes).

3cbe96d24c Revamp Profiler2 to make it more usable.
Features include new graphs to compare runtime of functions and runtimes
across reports, as well as new profiling functions that only profile
spikes.

7ada7dd2a8 Some tweaks to profiler2:
* separate the HTTP profiling server from the on-screen profiler
* allow shutting down the HTTP profiler
* print messages when enabling/disabling HTTP and GPU profilers
