* README.win32: Some clarifications.

* app/makefile.{cygwin,msc}

* plug-ins/makefile.{cygwin,msc}: Changes corresponding to the GTk+
source reorg. Add new files.

* app/text_tool.c: Remove now unnecessary workaround for Win32
POINTS identifier clash.
